Open an image 640 x 480, transparent.
Select a foreground and a background color that you want your frame to be.
Select the rectangle from the preset shapes tool from your left side tool bar

Start at 10,10 (look down in your left hand corner and watch the numbers there.
Make the rectangle the size of your image.

With your magic wand, select the colored part of the rectangle. 
Tolerance = 0 Match Mode= RGB Value
While the marching ants are running around your rectangle,
Go to Image, Effects, Inner Bevel and use these settings.

Selections, Select None.
Add a new layer.
Switch your background color and make it the foreground color
Go to your Preset Shapes, select Retangle again using these settings.

put in your second rectangle . starting at 50,50.
With your magic wand, select the new rectangle.
GO up to Image, Effects, Inner Bevel and apply with these settings.

With your magic wand select the inside of the rectangle on layer 2.
Go to Selections, Invert.
Go to Image, Effects, Drop Shadow and add it with these settings.

Add another drop shadow with the same setting except using a -2, -2.
Add a new layer. Switch your foreground and background color to where the lighter color
is your foreground color again..
Using your preset shapes, rectangle, use these settings and start at 100,100
Draw the rectangle to where it touches the opposite bottom corner.

With your magic wand, click the center of this rectangle, 
go to Selections, Invert.
Now we are going to add a Drop shadow using the same settings as we did above.
Go to Selections, Select None.
Go into your layer palette and move layer 3 under layer 2

Your image should now look something like this.

You might want to save this right now as a templet or just because it is best to save.
Minimize this image for now, we will come back to it later.
